Neoadjuvant ivonescimab: pCR 51% with chemo in resectable NSCLC

A non-randomized phase II study of neoadjuvant ivonescimab in resectable NSCLC (Nature Medicine) showed MPR 60% and pCR 30% with ivonescimab alone (n=10), and MPR 70% and pCR 51% with ivonescimab plus chemotherapy (n=67; pCR 61% in squamous), with 1 treatment-related surgery delay. Ivonescimab is investigational. The KEYNOTE-867 publication in JCO added an OS HR of 1.33 (0.93–1.90), numerically worse with pembrolizumab added to SBRT in stage I–II NSCLC, with 5 treatment-related deaths, all in the pembrolizumab arm.

“Wow! If such high pCR rates are replicated in larger trials, we next need to identify reliable algorithms to predict pCR and determine whether there are patients in whom we can omit surgery.”

IKS014: ORR 50% after T-DXd in HER2+ MBC

In an abstract to be presented at SABCS 2026, the HER2 ADC IKS014 (caxmotabart entudotin) showed an ORR of 50% (5/10) in HER2-positive metastatic breast cancer previously treated with T-DXd. IKS014 is investigational. Dr. Paolo Tarantino placed the result alongside earlier payload-switch data from DB02 and TULIP.

UNICORN: neoadjuvant botensilimab + balstilimab, MPR 36% in pMMR colon cancer

The phase 2 UNICORN trial (Nature Medicine) gave short-course neoadjuvant botensilimab, an Fc-enhanced anti-CTLA-4, with or without the anti-PD-1 balstilimab, to patients with resectable T3–4 colon cancer in four cohorts of 14 (N=56). In pMMR disease, major pathological response was 0% with botensilimab alone and 36% with the combination (pCR 29%); in dMMR disease, the combination gave MPR 100% and pCR 93%. Both drugs are investigational, the cohorts are small and non-randomized, and survival follow-up is immature.

ENERGIZE misses both co-primary endpoints in MIBC

The phase 3 ENERGIZE trial (CA017-078, n=653) of neoadjuvant nivolumab plus gemcitabine-cisplatin followed by adjuvant nivolumab in cisplatin-eligible MIBC met neither co-primary endpoint: EFS HR 0.94 and pCR 39.1% vs 29.6%, not statistically significant (Annals of Oncology). Mature THOR data for erdafitinib vs chemotherapy in metastatic urothelial cancer showed OS 12.1 vs 8.7 months (HR 0.65) and ORR 46.9% vs 12.6%. The investigational ENPP3×CD3 T-cell engager XmAb819 showed a confirmed ORR of 45.5% (5/11) at 400 µg/kg in heavily pretreated ccRCC.

Relapsed myeloma: guidelines prefer bispecifics or CAR-T

Dr. Aaron Goodman wrote that the guidelines' preferred options for relapsed myeloma are bispecific T-cell engagers or CAR-T, and called for an end to NCI trials randomizing patients to what he described as substandard therapy. Dr. Breno Moreno de Gusmão proposed a four-state framework for joint MRD and PET reporting. Dr. Robert Orlowski highlighted a UK NHS real-world study in which NGS re-classified 2% of newly diagnosed patients as high-risk.

Bispecifics in CNS lymphoma: a persistent effectiveness signal

Dr. Jose Sandoval described a persistent signal of effectiveness for bispecific antibodies in CNS lymphomas, with prospective trials still pending. A meta-analysis examined survival by phased-variant ctDNA and PET response in large B-cell lymphoma. Dr. Aaron Goodman wrote that it is increasingly difficult in oncology to determine whether a cancer therapy is futile.
