View post ↗Full Lung Cancer coverage in today's digest →The phase III KEYNOTE-867 trial (JCO) of SBRT with or without pembrolizumab in inoperable stage I–II NSCLC (n=448) showed median event-free survival of 31.2 vs 28.3 months (HR 0.92), with grade 3–5 treatment-related AEs of 20.4% vs 3.7% and all five grade 5 events in the pembrolizumab arm before the trial stopped early. Dr. Aakash Desai's map of 47 active lung cancer trials (Clinical Lung Cancer) found bispecific antibodies make up about 70% and ADCs about 30%, concentrated on EGFR×c-MET, TROP2 and HER2. In her ESMO 2026 recap, Dr. Isabelle Opitz noted that SARON improved progression-free survival with radiotherapy in driver-negative oligometastatic NSCLC (13.9 vs 8.9 months) without an overall survival benefit.

“Latest update to @ASCO Living Guidelines from @Joshua_Reuss et al reaffirms first line lorlatinib for patients with advanced ALK+ NSCLC with use of alectinib, brigatinib, or ensartinib only when lorlatinib not suitable or available.”

An updated meta-analysis of adjuvant CDK4/6 inhibitors in high-risk HR+/HER2− breast cancer found improved iDFS and DRFS with no OS benefit and higher rates of grade ≥3 adverse events. SABCS 2026 abstract titles are now available, from a record 2,300+ submissions.

Dr. Pashtoon Kasi described the results as corroborating the NEST trial. A Cancer Cell multiomic study of microsatellite-stable colorectal cancer from the pan-EU COLOSSUS project describes distinct biology and targetable vulnerabilities.

“UNICORN reports 100% DFS as well in pMMR colorectal cancer.”

The PROTEUS substudy of perioperative apalutamide plus ADT vs radical prostatectomy alone in high-risk or locally advanced prostate cancer (n=408) showed 3-year event-free survival of 63% vs 41% (HR 0.38), while metastasis-free survival by conventional imaging alone was similar (94% vs 92%). ENZARAD (JCO, n=802) found that 2 years of enzalutamide added to RT plus ADT did not improve 8-year metastasis-free survival (74% vs 72%, HR 0.88, P=0.34), with a hypothesis-generating signal in the prespecified pelvic nodal RT subgroup (HR 0.47). Dr. Tony Felefly argued that node-negative patients benefited from intensification in STAMPEDE and DASL-HiCaP, where nodal status was a stratification factor.

“So N0 clearly benefited from intensification.”

A Blood Cancer Journal study with MSK's Geriatric Service examined graded frailty assessment and outcomes after CAR-T in relapsed/refractory multiple myeloma. The FASTER study, comparing a bispecific antibody with transplant in newly diagnosed myeloma, is enrolling.

“Is a bispecific antibody better than transplant for newly diagnosed #myeloma patients?”
— @HadidiSamer · FASTER study · View post ↗A retrospective real-world series of 47 patients with relapsed/refractory classical Hodgkin lymphoma showed a 61% complete response rate with PD-1 inhibitor plus chemotherapy vs 10% with a PD-1 inhibitor alone, and 97% complete response after autologous transplant (Haematologica). In 138 matched patients with large B-cell lymphoma, immune recovery was slower and severe infections more frequent after CAR19 than after autologous transplant. In acute myeloid leukemia, eltrombopag started after chemotherapy washout did not improve survival but reduced the need for platelet transfusions.

“Retrospective, but it fits the trial data: PD-1 + chemo is a strong bridge to transplant in 2L.”
