CLARITY-Gastric01 Trial

CLARITY-Gastric01 (NCT06346392) tested AstraZeneca’s investigational CLDN18.2 antibody-drug conjugate sonesitatug vedotin (Sone-Ve) against investigator’s choice in previously treated CLDN18.2-positive gastric/GEJ cancer. At ESMO 2026 (LBA7), one dual primary endpoint was met, OS in 3L+ (10.0 vs 5.0 months; HR 0.47), and the other, central-review PFS in all 2L+ patients, was not (HR 0.77; P=0.053).

CLARITY-Gastric01 Key Takeaways

Endpoint hierarchy: what was tested, in whom

CLARITY-Gastric01 had two dual primary endpoints: overall survival in the 3L+ population (patients with ≥2 prior lines; n=355) and PFS by blinded independent central review (BICR) in the ITT population (all randomized 2L+ patients; n=532). OS in the ITT population was the key secondary endpoint. The multiple testing procedure gave ITT PFS an initial two-sided alpha of 0.1% and 3L+ OS 4.9%, with alpha recycling to ITT OS (design slide). Result: 3L+ OS met; ITT PFS not met; ITT OS met. All results are from a planned interim analysis (press release) with data cutoff May 31, 2026 (abstract; press release) and median follow-up of 8.6 months (abstract).

Dual primary endpoint met: overall survival in 3L+

In patients treated in the third line or later, median OS was 10.0 months (95% CI 7.2–11.4) with Sone-Ve vs 5.0 months (4.2–6.4) with investigator’s choice: HR 0.47 (0.36–0.62), with 85 vs 122 deaths. 12-month OS was 38.1% vs 18.1%. The P-value is printed as <0.000001 on the slide and in the press release and as <0.001 in the abstract (DCO May 31, 2026).

Dual primary endpoint not met: PFS by central review in all 2L+ patients

In the ITT population, median PFS by BICR was 4.2 months (3.5–5.3) vs 3.8 months (3.0–4.2): HR 0.77 (0.62–0.96), P=0.053193 on the slide and in the press release (P=0.05 in the abstract), which did not reach the threshold for statistical significance. In the 3L+ population (secondary endpoint, not formally tested), PFS by BICR was 4.1 vs 2.7 months, HR 0.56 (0.43–0.74). Investigator-assessed PFS in ITT, an exploratory endpoint, was 4.4 vs 3.0 months, HR 0.59 (0.48–0.73) (DCO May 31, 2026).

Key secondary endpoint: overall survival in all 2L+ patients (ITT)

Median OS was 10.6 months (9.1–12.0) vs 6.4 months (5.8–7.6): HR 0.58 (0.47–0.73), with 139 vs 174 deaths; 12-month OS 42.6% vs 27.8%. The P-value is printed as 0.000004 on the slide, <0.000004 in the press release and <0.001 in the abstract (DCO May 31, 2026). About two thirds of the ITT population was 3L+. In the prespecified 2L subgroup (descriptive; confidence intervals not adjusted for multiplicity), the OS HR was 0.88 (0.60–1.30), with 54/89 vs 52/88 deaths; in 3L+ it was 0.47.

OS by CLDN18.2 expression, response

OS in the ITT population favoured Sone-Ve in both prespecified expression subgroups: CLDN18.2 ≥75%, 10.6 vs 6.4 months, HR 0.59 (0.45–0.78); ≥25% to <75%, 10.5 vs 6.2 months, HR 0.63 (0.42–0.94). Among patients with measurable disease (189 per arm), confirmed ORR by BICR was 23.3% vs 13.8% (risk difference 10.2; 95% CI 2.68–17.83), complete responses 3.7% vs 0.5%, disease control 70.4% vs 55.6%, and median duration of response 8.5 vs 6.8 months (response slide). The expression-subgroup HRs are on the slide, in the abstract and in the press release (DCO May 31, 2026).

Regulatory status & clinical implications

⚠ Investigational. Sonesitatug vedotin is not approved by the FDA or any other regulator; it holds FDA and European Commission Orphan Drug Designation for gastric and GEJ cancers and Breakthrough Designation in China for 2nd-line gastric cancer (AstraZeneca). ✓ The only FDA-approved CLDN18.2-directed agent is zolbetuximab (VYLOY, Astellas), approved October 18, 2024 for first-line use with fluoropyrimidine- and platinum-containing chemotherapy in HER2-negative, CLDN18.2-positive gastric or GEJ adenocarcinoma, with the VENTANA CLDN18 (43-14A) RxDx Assay as companion diagnostic. CLARITY-Gastric01 used a broader CLDN18.2 definition (≥25% of tumour cells at any intensity), which AstraZeneca says goes beyond the current definition of CLDN18.2 positivity; the Ventana SP455 assay is being evaluated for Sone-Ve. Only 17 randomized patients (10 vs 7) had prior zolbetuximab, so activity after zolbetuximab remains unknown. The discussant, Salah-Eddin Al-Batran, MD, concluded: “≥3L: Impressive efficacy in a setting of high unmet need”; “2L: Potential benefit, but more data needed, particularly versus ramucirumab/paclitaxel”; and “Combining 2L and ≥3L complicates clinical interpretation.” First-line Sone-Ve is being tested in CLARITY-Gastric02.

Slide text (transcribed from the slide images) [Conclusions, presenter Rui-Hua Xu] CLARITY-Gastric01 is the first global, Phase 3 trial to demonstrate a statistically significant OS benefit and a manageable safety profile with a CLDN18.2 targeting antibody-drug conjugate in patients with previously treated advanced or metastatic CLDN18.2-positive GEC (defined as ≥25% tumour cells with membraneous staining at any intensity) Sone-Ve showed a statistically significant and clinically meaningful improvement in OS versus investigator's choice of therapy • Median OS was doubled to 10 months in the 3L+ population and improved by over 4 months to 10.6 months in the ITT (2L+) population • OS benefit was maintained across CLDN18.2 expression subgroups PFS by BICR in ITT (HR 0.77) did not reach the threshold for statistical significance • PFS by BICR in 3L+ was improved with Sone-Ve versus investigator's choice of therapy (HR 0.56) • Assessment of PFS by investigator in ITT showed a more pronounced treatment effect (HR 0.59) than by BICR Sone-Ve demonstrated a differentiated and manageable safety profile versus investigator's choice of therapy Sone-Ve is a potential new standard of care in the 2L+ setting for patients with advanced or metastatic CLDN18.2-positive gastroesophageal carcinoma [My Conclusions, invited discussant Salah-Eddin Al-Batran] Sone-Ve represents an important therapeutic advance in gastroesophageal carcinoma. • ≥3L: Impressive efficacy in a setting of high unmet need. • 2L: Potential benefit, but more data needed, particularly versus ramucirumab/paclitaxel. • Study design: Combining 2L and ≥3L complicates clinical interpretation. • Safety: Manageable, but GI toxicity remains relevant. • Future: First-line trials ongoing; curative setting should be a next step [Overall Survival in Third-Line and Later Setting, discussant Salah-Eddin Al-Batran] Median overall survival (months), control arm | experimental arm, randomized trials ordered by year of primary report, as drawn on the discussant's chart: Apatinib 2016: 4.7 | 6.5; ATTRACTION-2 2017: 4.1 | 5.3; JAVELIN Gastric 300 2018: 5.0 | 4.6; TAGS 2018: 3.6 | 5.7; INTEGRATE IIa 2024: 4.0 | 4.5; CLARITY-Gastric01 2026: 5.0 | 10.0 Callout: "CLARITY-Gastric01 is a real step forward for our patients" (Cross-trial comparison across different populations and eras, as presented by the discussant.)

Abstract LBA7 (released before the presentation)
Oct 24, 2026 · ESMO 2026 · LBA7 abstract
View post on X ↗
Slide text (transcribed from the slide images) [LBA7 abstract, as released on the ESMO 2026 programme site (screenshots)] Background: CLDN18.2 is a validated therapeutic target in advanced GEC. Sone-Ve, a CLDN18.2-targeting monomethyl auristatin E–based antibody drug conjugate, demonstrated clinical activity in phase 1/2 studies of advanced GEC. Methods: CG-01 is a randomised, open-label, global, phase 3 study of Sone-Ve vs investigator's choice of therapy (INV) after ≥1 prior therapy for GEC expressing CLDN18.2 in ≥25% tumour cells with membranous staining at any intensity. [...] Dual primary endpoints are OS in 3L+ and progression-free survival (PFS) by BICR in ITT. Key secondary endpoint is OS in ITT. Table: 3L+ Sone-Ve (n=178) vs INV (n=177): median OS 10.0 (7.2–11.4) vs 5.0 (4.2–6.4) mo, HR 0.47 (0.36–0.62), p<0.001. ITT (2L+) (n=267 vs 265): median OS 10.6 (9.1–12.0) vs 6.4 (5.8–7.6), HR 0.58 (0.47–0.73), p<0.001; median PFS by BICR 4.2 (3.5–5.3) vs 3.8 (3.0–4.2), HR 0.77 (0.62–0.96), p 0.05. SAS (n=261 vs 252): TRAE any 224 (85.8) vs 205 (81.3); grade ≥3 95 (36.4) vs 113 (44.8); gastrointestinal toxicity 29 (11.1) vs 16 (6.3); haematology 20 (7.7) vs 40 (15.9); leading to death 3 (1.1) vs 2 (0.8); leading to discontinuation 8 (3.1) vs 11 (4.4). Conclusions: CG-01 showed a significant OS benefit with Sone-Ve vs INV in 3L+ and ITT, with a manageable safety profile. These findings support Sone-Ve as a potential 2L+ treatment for a broad patient population with advanced CLDN18.2-positive GEC.

Sonesitatug vedotin reduced the risk of death by 42% in 2nd- and later-line CLDN18.2-positive advanced gastric/GEJ cancers in CLARITY-Gastric01 - AstraZeneca https://t.co/CouYIPpwrz

Five Clinical Questions Behind the ESMO 2026 Headlines CLARITY reports an OS benefit, but its broader PFS endpoint was not significant. KEYNOTE-C93 has a positive PFS readout with immature OS. MOIO asks whether responders need less frequent immunotherapy. The details will matter more than the headlines. #ESMO26

CLARITY-Gastric01 Trial Design

CLARITY-Gastric01 asked whether an antibody-drug conjugate aimed at CLDN18.2 could improve survival after first-line therapy for advanced gastroesophageal adenocarcinoma, using a broader CLDN18.2 definition than earlier trials. AstraZeneca is the sponsor; the trial runs at 175 centres in 19 countries (press release). Results were presented by Rui-Hua Xu, MD, at ESMO 2026 (LBA7, Presidential Symposium II, October 25, 2026), with Salah-Eddin Al-Batran, MD, as invited discussant.

Study Design

Randomized, open-label, sponsor-blinded, global phase 3 (NCT06346392). Initially 1:1:1 to Sone-Ve 2.2 mg/kg, Sone-Ve 1.8 mg/kg or investigator’s choice; the 1.8 mg/kg arm closed after dose selection (January 2025) and randomization continued 1:1. 594 enrolled; efficacy compares 267 vs 265. Stratified by region, prior gastrectomy and line of therapy.

Population

Unresectable locally advanced or metastatic gastric, GEJ or oesophageal adenocarcinoma after ≥1 prior therapy, CLDN18.2 in ≥25% of tumour cells with membranous staining at any intensity. Median age 57; about one third 2L, two thirds 3L+; CLDN18.2 ≥75% in about 71%; prior zolbetuximab in 17 patients.

Interventions

Sone-Ve 2.2 mg/kg IV every 3 weeks. Control at 2L: ramucirumab + paclitaxel, or paclitaxel or docetaxel if ramucirumab contraindicated; at 3L+: irinotecan, trifluridine/tipiracil (except China) or apatinib (China only).

Endpoints

Dual primary: OS in 3L+; PFS by BICR in ITT (all 2L+). Key secondary: OS in ITT. Other: PFS by BICR in 3L+, ORR and DoR, safety, OS by CLDN18.2 expression, investigator-assessed PFS/ORR/DoR, OS in prespecified subgroups.

Presenter & Discussant

Presenter and principal investigator: Rui-Hua Xu, MD, PhD (Sun Yat-sen University Cancer Center, Guangzhou). Invited discussant: Salah-Eddin Al-Batran, MD (Frankfurt am Main). Session chairs: Giuseppe Curigliano and Yelena Y. Janjigian.

Data Cut

Planned interim analysis; data cutoff May 31, 2026; median follow-up 8.6 months.

CLARITY-Gastric01 Questions

What is the CLARITY-Gastric01 trial?

CLARITY-Gastric01 (CG-01, NCT06346392) is a global, randomized, open-label, sponsor-blinded phase 3 trial sponsored by AstraZeneca. It compared sonesitatug vedotin (Sone-Ve, AZD0901), an investigational CLDN18.2-directed antibody-drug conjugate with an MMAE payload, with investigator’s choice of therapy in previously treated advanced gastric, gastroesophageal junction or oesophageal adenocarcinoma expressing CLDN18.2 in at least 25% of tumour cells at any intensity. Design: Trial Design.

Did CLARITY-Gastric01 meet its primary endpoints?

One of its two dual primary endpoints was met. Median overall survival in the 3L+ population was 10.0 vs 5.0 months (HR 0.47; 95% CI 0.36–0.62). Median PFS by blinded central review in all randomized 2L+ patients was 4.2 vs 3.8 months (HR 0.77; 95% CI 0.62–0.96; P=0.053) and did not reach statistical significance. Median OS in all 2L+ patients, the key secondary endpoint, was 10.6 vs 6.4 months (HR 0.58; 95% CI 0.47–0.73). Figures: At a Glance.

Did Sone-Ve help in the second-line setting?

That is not settled. Second-line patients were about a third of the trial. In the prespecified but descriptive 2L subgroup, the OS hazard ratio was 0.88 (95% CI 0.60–1.30), with confidence intervals not adjusted for multiplicity. The discussant, Salah-Eddin Al-Batran, MD, concluded that the 2L benefit is potential but needs more data, particularly versus ramucirumab plus paclitaxel, and that combining 2L and 3L+ complicates interpretation. See Discussion Threads.

Does Sone-Ve work after zolbetuximab?

The trial cannot answer that yet. Only 17 randomized patients had received prior zolbetuximab (10 in the Sone-Ve arm, 7 in the control arm), and no separate result was presented for them. Physicians raised whether CLDN18.2 status should be re-tested by biopsy after earlier treatment.

Is sonesitatug vedotin approved?

No. Sone-Ve is investigational and not approved by the FDA or any other regulator; it has FDA and European Commission Orphan Drug Designation for gastric and GEJ cancers. The only approved CLDN18.2-directed agent is zolbetuximab (VYLOY, Astellas), approved by the FDA on October 18, 2024 for first-line use with fluoropyrimidine- and platinum-containing chemotherapy in HER2-negative, CLDN18.2-positive gastric or GEJ adenocarcinoma, with the VENTANA CLDN18 (43-14A) RxDx Assay as companion diagnostic.

How did safety compare?

Grade ≥3 treatment-related adverse events occurred in 36.4% with Sone-Ve vs 44.8% with investigator’s choice. Gastrointestinal adverse events (any grade, all causes: nausea 54.4% vs 26.6%, vomiting 41.0% vs 24.6%) were more common with Sone-Ve and haematologic toxicity less common. Treatment-related deaths were 1.1% vs 0.8%. Details: At a Glance.

Oct 25, 2026 ● POSITIVE The Trojan horse was successful. In patients with pretreated and metastatic gastric/esophageal cancer sonesitatug-vedotin doubled survival to 10.0 versus 5.0 months with standard chemo, risk of death down 53%. And it worked even in tumors with lower levels of the target label, widening eligibility to around 60% of patients in this setting, not a narrow slice. In a cancer where later line treatment has meant 5-9 months for my whole career, doubling that isn't small. It’s a great success for a majority of patients, and there are more ADCs coming, also with Claudin18.2 as a target.

Sonesitatug vedotin reduced the risk of death by 42% in 2nd- and later-line CLDN18.2-positive advanced gastric/GEJ cancers in CLARITY-Gastric01 - AstraZeneca https://t.co/CouYIPpwrz

Benefit even in patients with lower CLDN18.2 expression is encouraging. However, the primary PFS endpoint was not met in the ITT population. Its role in 2L and efficacy after zolbetuximab remain unclear. one question: Given that CLDN18.2 expression can change after treatment, should a fresh biopsy have been mandatory at study enrollment? Or does lowering the positivity cutoff to ≥25% reduce the clinical relevance of this discordance, making archival tissue sufficient?
