DASL-HiCaP (ANZUP1801) is an international phase 3 trial adding darolutamide (NUBEQA, Bayer) to ADT and radiotherapy in very high-risk localised prostate cancer (N=1,106). At ESMO 2026 the interim analysis met its primary endpoint: 4-year metastasis-free survival 95% vs 88% (HR 0.42). Overall survival is immature; this use is investigational.
See the ESMO 2026 results →Posts by physicians about DASL-HiCaP during ESMO 2026 week (October 24–25, 2026), after the LBA4 abstract was released and Tamim Niazi presented the interim analysis in Presidential Symposium I. Each post is shown in full and verbatim, newest first, and linked to the original on X.
𝕏Congratulations to the DASL-HiCaP research team on these very promising initial results. However, I must be emphasized that this is a planned interim analysis, triggered when the primary endpoint MFS-CI crossed the pre-specified statistical boundary (HR 0.42; p = 0.0001), rather than the final analysis. Crucial secondary endpoints, such as OS and PCSS, remain immature in terms of data; therefore, the trial requires further follow-up. Furthermore, the current median follow-up is only about 4 years, whereas the regimen only administers ADT and Darolutamide for 2 years (96 ws). So, the post-treatment period is not yet long enough to determine whether the treatment efficacy is truly durable and maintained long-term, or if it is merely a temporary suppressive effect. @TonyFelefly @TylerSbrt @jryckman3
𝕏Good to have an alternative to adjuvant abiraterone for very high-risk localized prostate cancer. DASL also included early salvage RT after prostatectomy, which matters. OS is still immature, but the early MFS data look impressive. Comorbidities and drug interactions will still drive the choice. #ESMO2026 #prostatecancer

𝕏Results from the DASL-HiCaP study were presented on October 24, 2026. DASL-HiCaP (ANZUP1801) – ESMO 2026 Study objective: To evaluate the addition of darolutamide to standard androgen deprivation therapy (ADT) and radiotherapy in patients with very high-risk localized prostate cancer. Phase III: 1,106 patients; randomized, double-blind, placebo-controlled. Treatment: Darolutamide 600 mg twice daily for 96 weeks + ADT and radiotherapy. Control: Placebo + ADT and radiotherapy. Median follow-up: 3.8 years. Primary endpoint: Imaging-based metastasis-free survival (MFS-CI) was significantly improved: HR 0.42 (95% CI 0.27–0.66; p=0.0001). The risk of PSA progression or death was reduced by 53%: HR 0.47 (95% CI 0.33–0.67). Overall survival: The difference was not yet statistically significant: HR 0.88 (95% CI 0.49–1.59). Safety Grade ≥3 adverse events occurred in 43% of patients in the darolutamide group and 36% in the placebo group. Grade ≥2 fatigue/asthenia was reported in 14% and 10% of patients, respectively. Clinical Take-Home Message Darolutamide + ADT + radiotherapy increased 4-year metastasis-free survival from 88% to 95% in patients with very high-risk localized prostate cancer. This represents an absolute improvement of 7 percentage points and a 58% relative reduction in the hazard of metastasis or death. These findings support treatment intensification in this patient population. However, as overall survival data remain immature, an overall survival benefit should not yet be considered established. 𝕏2/n All patients received comprehensive, contemporary prostate and nodal RT. There was some contention that in the STAMPEDE trial, had nodal RT (or higher dose prostate-directed RT) been mandated, it might have reduced the benefit of Abi. Here that argument is moot. Further, the MFS benefit in Darolutamide was present regardless of PSMA PET staging, regardless of nodal status.
𝕏3/n Finally, as to the choice of ARPIs, I would argue Daro is substantially better tolerated than Abiraterone. The side effect profile reported by the team today is consistent with that conclusion.
𝕏1/n Major congrats to our DASL team, especially to @ChrisSweens1 and @niazi_Dr7, and a big thanks to the patients. 𝕏Darolutamide combined with radiotherapy significantly prolonged metastasis-free survival in patients with high-risk localized prostate cancer. 𝕏Putting STAMPEDE, ENZARAD, PROTEUS & DASL into context with the latest #ESMO26 updates: STAMPEDE: Abiraterone-based intensification improved MFS AND OS, predominantly with RT. New #ESMO26 Decipher data suggests differential abi benefit in cN0 disease (interaction P=.01); using this to omit treatment still requires validation. ENZARAD: At 8 years, MFS was 74% vs 72% (HR 0.88; P=.34). No significant overall MFS benefit from enza + ADT/RT. The cN1/planned pelvic RT subgroup signals remain hypothesis-generating. DASL: new at #ESMO26: 1,106 patients; 96 weeks of daro v. placebo + ADT + prostate/bed AND pelvic RT. Four-year conventional-imaging MFS: 95% vs 88% (HR 0.42). Very-high-risk population, including selected postoperative patients. Interim analysis; OS immature. PROTEUS main trial: Perioperative apa + ADT improved 5-year PSMA-inclusive MFS versus ADT alone: 78.2% vs 73.5% (HR 0.80). Conventional-imaging MFS was not statistically significant. #ESMO26 post hoc PSA-response analysis adds prognostic information. PROTEUS randomized substudy: new at #ESMO26: Perioperative apa/ADT + RP/PLND (n=102) v. immediate RP/PLND (n=306). At 37.8 months’ follow-up: • 3-year EFS: 63.0% vs 41.4%; HR 0.38. • Secondary PSMA-inclusive MFS: HR 0.49; nominal P=.0193. • Subsequent postoperative RT: 20% vs 43%. The substudy strengthens a "perioperative strategy". But conventional-imaging entry followed by PSMA-inclusive endpoints matters. PSA-based EFS, conventional MFS and PSMA-inclusive MFS are different endpoints. These trials support specific intensification strategies; they cannot rank the drugs or establish RP-versus-RT superiority. The decision remains who benefits enough to justify the ENTIRE treatment burden.

DASL-HiCaP (Darolutamide Augments Standard therapy for Localised very High-risk Cancer of the Prostate; ANZUP1801; NCT04136353) is an investigator-initiated, international, randomised, double-blind, placebo-controlled phase 3 trial. It asked whether adding darolutamide to the standard of 96 weeks of ADT plus radiotherapy to the prostate or prostate bed and pelvic nodes improves outcomes in very high-risk localised prostate cancer, or in persistent/rising PSA within 1 year after radical prostatectomy.
Tamim Niazi (McGill University / Jewish General Hospital, Montreal) presented the pre-specified interim analysis as LBA4 in Presidential Symposium I at ESMO 2026 on October 24, 2026; Silke Gillessen was the invited discussant. DASL-HiCaP (ANZUP1801, NCT04136353) is an international, double-blind phase 3 trial that randomised 1,106 men with very high-risk localised prostate cancer, or persistent/rising PSA soon after prostatectomy, to darolutamide or placebo for 96 weeks on top of 96 weeks of ADT and prostate/pelvic radiotherapy. See Trial Design.
At the pre-specified interim analysis, darolutamide met the primary endpoint, metastasis-free survival by conventional imaging (4-year 95% vs 88%, HR 0.42, p=0.0001). Overall survival is immature. All figures with their source are in At a Glance.
No. Use with ADT and radiotherapy in this setting is investigational. Darolutamide (NUBEQA) is FDA-approved for non-metastatic castration-resistant and metastatic castration-sensitive prostate cancer.
Not yet shown. At the interim analysis there were 21 vs 24 deaths (OS HR 0.88, 95% CI 0.49–1.59) and follow-up is continuing; the investigators said follow-up will confirm the surrogacy of MFS-CI for overall survival.
The benefit was seen across pre-specified subgroups. The invited discussant, Silke Gillessen, said the clearest indication is node-positive (N1) disease and that for N0 disease the indication is less clear. Subgroup figures are in At a Glance.
Any grade ≥3 adverse events occurred in 43% with darolutamide vs 36% with placebo. Rates of adverse events of special interest are in At a Glance.
