HARMONi-GI1 (NCT06591520) is a phase 3 trial in China comparing ivonescimab, Akeso’s investigational PD-1/VEGF bispecific antibody, plus gemcitabine-cisplatin with durvalumab (IMFINZI) plus gemcitabine-cisplatin in 682 patients with untreated advanced biliary tract cancer. At ESMO 2026 (LBA8), overall survival improved: 14.9 vs 12.1 months, HR 0.75, at a prespecified interim analysis.
See the LBA8 Results →At the prespecified interim analysis (median follow-up 14.3 months), median OS was 14.9 months (95% CI 12.9–not estimable) with ivonescimab plus gemcitabine-cisplatin vs 12.1 months (11.0–13.4) with durvalumab plus gemcitabine-cisplatin: stratified HR 0.75 (95% CI 0.61–0.92), one-sided P = 0.0031. OS rates were 59.7% vs 50.7% at 12 months and 44.0% vs 32.5% at 18 months (LBA8 slides and abstract, interim analysis). The abstract prints the P value without stating sidedness; the slides and the Summit release give it as one-sided. The invited discussant’s slide puts the interim analysis at 81.6% maturity. The data cutoff date was not shown on the slides or in the releases.

LBA8 presentation slides (Jian Zhou, ESMO 2026) · Akeso press release, Oct 25, 2026⚠ Investigational. Ivonescimab is not approved for biliary tract cancer in any country. In China it is approved by the NMPA in three lung cancer indications; Akeso says ivonescimab plus chemotherapy for first-line BTC has been proposed for NMPA Priority Review (the presenter’s slide lists it as “Priority Review designation”). In the United States and Europe ivonescimab is not approved for any indication (Summit release); its first US application, in EGFR-mutant NSCLC after a third-generation EGFR inhibitor, is a separate indication covered on our HARMONi-A page. ✓ The comparator, durvalumab (IMFINZI) plus gemcitabine-cisplatin, is an approved first-line standard based on TOPAZ-1. The presenter’s conclusion slide states that HARMONi-GI1 “supports ivonescimab plus chemotherapy as a new standard of care for first-line BTC”; the abstract describes it as “a potential new first-line standard of care.” Neither is a regulatory decision. The trial enrolled patients in China only; in the US, ECOG-ACRIN’s registration-enabling phase 2/3 trial EA2251 (NCT07765433) will compare ivonescimab plus chemotherapy with pembrolizumab or durvalumab plus chemotherapy.

We already know that ivonescimab is effective. However, something else caught my attention on this slide. In the control arm, 7% of patients received “Traditional Chinese Medicine” as subsequent antitumor therapy, compared with only 0.6% in the ivonescimab arm. What exactly are these treatments? Have they demonstrated any survival benefit, or is their efficacy no better than placebo? And the crucial question: Could this imbalance have influenced OS, given the substantially higher use in the control arm? Have similar treatments been reported in other randomized oncology trials? #ESMO26

#ESMO26 Ivonescimab!!! We should judge trials by the quality of their data and the benefit they bring to patients, not by their country of origin. Scientific scrutiny should follow the same standards for everyone; political biases must not delay patients’ access to effective treatments. HARMONi-GI1 asked whether replacing durvalumab with ivonescimab, a bispecific antibody that targets both PD-1 and VEGF, improves survival when added to gemcitabine and cisplatin as first-line treatment for advanced biliary tract cancer. Durvalumab plus gemcitabine-cisplatin is the standard first-line regimen. The trial is sponsored and funded by Akeso Biopharma (Zhongshan, China), which generated, managed and analyzed the data; Summit Therapeutics holds ivonescimab rights in the US, Europe and other territories. Results were presented by Jian Zhou, MD, PhD, at ESMO 2026 (LBA8, Presidential Symposium II, October 25, 2026), with James J. Harding, MD, as invited discussant.
Randomized 1:1, double-blind (each arm received placebo on the other arm’s maintenance schedule), multicenter phase 3 in China (NCT06591520, AK112-309); 340 vs 342 patients. Stratified by disease status (locally advanced vs metastatic) and primary tumor location (intrahepatic vs extrahepatic cholangiocarcinoma vs gallbladder).
Unresectable locally advanced or metastatic BTC, or recurrent after curative surgery; no prior systemic therapy (neoadjuvant or adjuvant therapy allowed if completed ≥6 months before progression); ECOG PS 0–1. Intrahepatic cholangiocarcinoma 58%; hepatitis B 59.6% (discussant’s slide).
Ivonescimab 20 mg/kg Q3W or durvalumab 1500 mg Q3W, each with gemcitabine 1000 mg/m² and cisplatin 25 mg/m² on days 1 and 8 every 3 weeks for up to 8 cycles, then ivonescimab Q3W or durvalumab Q4W maintenance.
Primary: overall survival. Key secondary: PFS and ORR (investigator-assessed, RECIST v1.1). Other secondary (registry): duration of response, disease control rate, time to response, adverse events.
Presenter: Jian Zhou, MD, PhD (Zhongshan Hospital, Fudan University, Shanghai), principal investigator. Invited discussant: James J. Harding, MD (New York).
Prespecified interim analysis, reviewed by an Independent Data Monitoring Committee according to Akeso; median follow-up 14.3 months; 81.6% maturity (discussant’s slide). The data cutoff date was not shown.
HARMONi-GI1 (AK112-309, NCT06591520) is a randomized, double-blind phase 3 trial run in China by Akeso. It compared ivonescimab, a PD-1/VEGF bispecific antibody, plus gemcitabine and cisplatin with durvalumab plus gemcitabine and cisplatin as first-line treatment in 682 patients with unresectable locally advanced or metastatic biliary tract cancer. Design: Trial Design.
At a prespecified interim analysis (median follow-up 14.3 months), the primary endpoint was met: median overall survival was 14.9 vs 12.1 months, HR 0.75 (95% CI 0.61–0.92), one-sided P = 0.0031. Median PFS was 8.4 vs 5.8 months (HR 0.61) and ORR 53.8% vs 32.5%. Figures: At a Glance.
No. Ivonescimab is not approved for biliary tract cancer anywhere. In China it is approved (NMPA) in three lung cancer indications; Akeso says first-line BTC has been proposed for NMPA Priority Review. In the United States and Europe ivonescimab is investigational and not approved for any use. Its first US application, in EGFR-mutant NSCLC, is a separate indication (see HARMONi-A).
Grade 3 or 4 treatment-emergent adverse events occurred in 82.0% vs 72.3% of patients, mostly hematologic; serious adverse events were 43.4% vs 43.7%, and grade ≥3 immune-related events 4.1% in each arm. Treatment exposure was longer with ivonescimab (median 7.8 vs 5.7 months). Details: At a Glance.
Yes, HARMONi-GI1 enrolled patients in China only. Physicians debated how far the results apply outside China (see Discussion Threads). In the US, ECOG-ACRIN plans a registration-enabling phase 2/3 trial, EA2251 (NCT07765433), comparing ivonescimab plus chemotherapy with pembrolizumab or durvalumab plus chemotherapy.
Fewer patients in the ivonescimab arm received any subsequent anti-tumor therapy (31.8% vs 45.9%). The slide also lists Traditional Chinese Medicine as subsequent therapy in 0.6% vs 7.0%, which Yakup Ergün, MD, questioned on X (see Physician Reactions).
